What is an Oophorectomy?

An oophorectomy is a surgical procedure to remove one or both ovaries. It is commonly performed using minimally invasive laparoscopic techniques and is usually completed as a day case under general anaesthetic. In some cases with large, complex or suspicious ovarian cysts, open abdominal surgery could be preferrable.

Oophorectomy may be recommended for several reasons, including:

  • Ovarian cysts or masses
  • Endometriosis affecting the ovaries
  • Ovarian torsion
  • Chronic pelvic pain
  • Risk-reducing surgery for certain genetic conditions
  • Suspected ovarian pathology

The procedure may involve removal of one ovary (unilateral oophorectomy) or both ovaries (bilateral oophorectomy). In many cases, it is performed alongside a salpingectomy (removal of the fallopian tubes).

What Happens During Surgery?

Oophorectomy is carried out under general anaesthetic.

During the procedure:

  • Small incisions are made in the abdomen
  • A laparoscope (camera) is inserted to provide a clear view of the pelvic organs
  • Surgical instruments are used to carefully detach and remove the ovary
  • If required, the fallopian tube may also be removed
  • The ovary is removed through one of the small incisions

If only one ovary is removed, the remaining ovary typically continues to produce hormones and release eggs. If both ovaries are removed, menopause will occur if it has not already happened.

Most patients return home the same day following monitoring in recovery.

Is Oophorectomy Right for Me?

Oophorectomy may be suitable if you:

  • Have persistent ovarian cysts or masses
  • Experience recurrent ovarian torsion
  • Have severe endometriosis affecting the ovaries
  • Have been advised to consider risk-reducing surgery
  • Have ongoing pelvic pain linked to ovarian pathology

If both ovaries are being considered for removal and you have not yet reached menopause, your surgeon will discuss hormonal implications and potential treatment options such as hormone replacement therapy.

A detailed consultation will assess your medical history, reproductive plans and overall health before recommending surgery.

If one ovary remains, hormone production typically continues normally.

Removal of both ovaries will trigger surgical menopause if you have not already gone through menopause. Hormone replacement therapy may be discussed.

Most patients return to light activities within a few days and resume normal routines within 2 to 4 weeks. In women when open surgery is recommended, normal activity is usually resumed within 6 weeks.
